Practicum & Independent Study Report


1. Fill out your portion of the Grade sheet and have your site supervisor fill out the appropriate part. Then make 3 copies. You keep a copy; give your site supervisor a copy; and give me a copy by the first week of the semester.

2. You will work 2 hours a week per credit hour you are registered for except during Summer semester. During Summer, you will work 3 hours a week per credit hour. Failure to complete the hours could result in an Incomplete to be made up the next semester that you are enrolled.

3. Keep the LOG SHEET up to date. Use the COMMENT SHEET to fully describe your daily routine for those who do the same thing daily and to describe unique situations/events/etc. Tell what is happening when you are there - what could be improved - what is good - how the practicum/independent study helped you. Run additional copies of these sheets as needed.

4. Turn your LOG SHEET, COMMENT SHEET, & GRADE SHEETS in to me no later than DEAD DAY (Study/ Reading Day). Failure to do this will result in an Incomplete until the materials are turned in or the grade automatically changed to Unsatisfactory.

5. Please remember: When you are at your volunteer site you are representing Auburn University and the Department of Kinesiology so you should act appropriately. Dress will depend of the location you are volunteering. However, if it is in a clinic, gym, hospital, or other type of professional facility, you are expected to dress professionally and/or abide by the guidelines set up by your site supervisor.

6. If you have questions or concerns at any time about any aspect of your Practicum or Independent Study, please contact me immediately. Use this opportunity to gain as much information as you can to assist you in your career path. Many students have made contacts through their experiences that have proven helpful when searching for jobs down the road. Do your best to make the most out of this experience!
